Please download the periodontal referral form, complete it, and email it to info@northcds.com.To help us assess each case efficiently, we also ask that all relevant patient images be included with the referral. 

Once we receive your submission, our team promptly reviews each submission and follows up with clear recommendations and next steps, so you always know where your patient stands in the treatment process.
